Is There Such a Thing as Good Posture? | Guardian Rebuttal So is there really such a thing as good posture? Well, good posture is a phrase that is perhaps less than ideal. We don’t talk about good blood pressure or good heart rate or good body temperature we talk about ‘ideal’ or ‘normal’ blood pressure, heart rate and body temperature. Just like there is a normal range for those vital signs, there is also an ideal for body alignment (posture) and an acceptable range of normal. So in answer to Luisa Dillner’s article in the Guardian: Are you sitting comfortably: the myth of good posture I say this: Let’s be very clear. There is a TON of research on alignment, spinal curves, spinal anomalies, degeneration etc. (Just pick up any copy of the Journal Spine, JMPT, Journal of Biomechanics, BMJ etc.) And when these factors deviate from normal, they change the ideal position of the body; away from ‘neutral’ alignment – where the body has a mechanical advantage against the forces of gravity and activities of daily living. Structure dictates function There is a well known axiom in anatomy, physiology and neurology that goes like this: Structures dictates function; or structure determines function. When the alignment of our spinal curves – in the sagittal, coronal and transverse planes – are altered, there exists the potential to influence general health. Take for example, Wolf’s Law by the German anatomist and surgeon Julius Wolff (1836–1902) in the 19th century; “states that bone in a healthy person or animal will adapt to the loads under which it is placed. That is why it is evidenced on spinal x-ray and MRI, that the areas of greatest spinal ‘misalignment’ (or in lay terms, poor posture), have the greatest degeneration of the discs, facet joints and vertebral bodies.” Let’s use the common ‘postural’ diagnosis hyperkyphosis (aka Dowager’s Hump) – where the ideal alignment (posture) of the thorax has been altered over time. A study in the J Orthop Sports Phys Ther (2010 June) Wendy B Katzman et al. Found the following: Functional Limitations “Excessive kyphosis has detrimental effects on physical performance, the ability to perform activities of daily living, and overall quality of life. Women with hyperkyphotic posture demonstrate difficulty rising from a chair repeatedly without using their arms, significantly poorer balance and slower gait velocity, wider base of support with stance and gait, and decreased stair-climbing speed—impairments that have been associated with increased risk for falls … have increased postural sway compared to those with normal posture.” Musculoskeletal Alterations “As kyphosis increases, there are concomitant alterations in the normal sagittal plane alignment that may cause pain and risk of dysfunction in the shoulder and pelvic girdle, and cervical, thoracic, and lumbar spine. Forward head posture, scapula protraction, reduced lumbar lordosis, and decreased standing height are often associated with hyperkyphosis. These postural changes increase the flexion bias around the hip and shoulder joints that can interfere with normal joint mechanics and movement patterns.” Free Posture Video Crash Course! Increased Risk of Fall Begins at 40! | Posture Doctor One of the main health concerns with aging is falling, which is often related to poor balance. In fact, many studies show that people begin to have balance problems starting at just 40 years of age! Balance Exercise for Beginners – 30 Ways to improve your balance in 30 days The older we get, the weaker our physical body and sensory abilities will be, which is why we need to consider our balance right now! In Japan, more than 7,000 people a year die from falling accidents, which already exceeds the number of traffic accidents. Test your balance You can determine how good your balance is by measuring the length of time that you can stand on one leg. The following table shows the average balance time by age group in a study conducted at a Japanese health institute. Average time with eyes open 20-39 years old: 110 seconds40-49: 64 seconds50-59: 36 seconds60-69: 25 seconds Average time with eyes closed 20-39 years old: 12 seconds40-49: 7 seconds50-59: 5 seconds60-69: less than 3 seconds If your balance time is below average, then you’ll have higher risk of falls, or slipping and tripping accidents. In the above study, women tend to lose their balance more than men but only by a small margin (1-2%). From this study, it is also evident that there’s a sudden significant decrease in the ability to maintain balance among middle-aged people (40 years and above – arg, that’s me!). The soles of our feet have sensors The skin throughout our body has a significant amount of  tiny pressure sensors or mechanoreceptors. Some areas have few pressure sensors, while other areas have thousands, like on the soles of your feet. The pressure sensors on the foot soles provide information to the brain to help balance our body. As we get older, the sensors get weaker and the soles of the feet lose sensitivity. Poor Blood Circulation Can Disrupt the Pressure Sensors People with poor blood circulation are twice as likely to be in a falling accident. Poor circulation leads to colder body temperatures, causing the pressure sensors on the soles of the feet, to lose sensitivity. The lost sensitivity is a little bit like soaking our feet in ice cold water for about 3 minutes and then trying to walk – can’t feel much! Pay attention to your forward-moving foot If our forward-moving foot hits something, our body will be off-balance and this can cause a stumble or fall. It is common sense to always have our eyes on path and watch where we are going but often we are miles away and lose track of the present moment. Walking is a great opportunity for being mindful – hence why many Buddhists practice walking meditation. Our lack of mindfulness is not the only problem. Our forward-moving foot is pointed down If our foot is pointed down while making a step, then we are more prone to falling. To avoid this, our forefoot or toes should be flexed upwards as shown in this image. This can be problematic, if like my mother, you have a foot drop. A foot drop is often the result of nerve impingement from a worn spinal disc (L5 to be exact). There are now simple devices that can be worn inside a sock, to help lift a foot drop during gait. 2. You walk like a pendulum. The height of your step can greatly increase your risk of falling. To prevent this, your forward-moving foot must be higher off the ground (at least 5 cm) while the knee is raised; as shown on the image below. When we get older, the balance information transmitted to our brain – from specialized nerve endings – communicates rather poorly, compared to when we were young. This leads to ineffective movements, and makes it hard to maintain the foot higher off the ground. How to Prevent Falls, Trips and Slips 1. Keep Your House Uncluttered There are a lot of things in our house that can contribute to clutter, and clutter can cause trips and falls. Always make sure to put away or store personal belongings and check for upturned carpet edges, children’s toys or dogs, in my case! 2. Stretch Your Feet and Ankles You might think that your feet do not need exercise or stretching compared to other parts of your body, but in reality, feet stretching exercise can really help your feet maintain balance. 3. Keep Your House Warm and Ensure Adequate Lighting Cold muscles and pressure sensors work less well and are less responsive to signals. Decreased temperature will also cause our muscles to have less strength and less flexibility, which can lead to accidents. Always try to keep your house warm or wear proper clothes and footwear, especially during winter. Since most falls occur indoors, make sure your house has adequate lighting. 4. I used to think nothing of taking daily aspirin, and I did that for a number of years. You think that you haven’t time to do anything about the oil, so you stick a piece of tape over the light. Now you can’t see the light and you feel better. Don’t you? Oil acts as a lubricant and prevents metal engine parts from coming in contact with other metal parts. If you didn’t change your oil or oil filter in your car, dirt would accumulate in the engine and the oil would lose its lubricating qualities and eventually the filter would stop working. The metal parts in your car would experience premature wear and eventually your engine would seize up and break down. Our bodies are a little bit like that. You can only ignore the warning signs for so long. When we take medication, we temporarily feel better because we don’t feel the pain, but the problem still exists. Don’t be fooled into believing that pain medication cures anything. Pain killers mask problems. Eventually the medication stops working and it becomes difficult to restore health. When we medicate, we fail to address the cause of the problem and therefore face the possibility of premature wear and chronic health issues. It’s tempting to medicate when we have pain, because of the hectic pace of our 24-hour lifestyle. Don’t wait for the pain. Start addressing your health and posture right now. Don’t mask your pain when it comes; and it will come! Learn to trust your body to heal and better still, ask for help. A functional or generic treatment approach (exercise classes, personal training, muscle strengthening) can never take altered spinal curves and straighten them out. A common mistake people make when trying to correct their posture is not understanding the real cause of poor posture. Many have tried several types of treatment and various exercise regimes in the past, but when they look in the mirror, they see little if any change. Does that sound familiar? The problem is, most gym trainers and exercise classes that claim to correct your posture, use only a functional approach. A functional approach works really well for someone who has very good body symmetry. In other words your left and right sides are mirror images of each other and there are no obvious imbalances. Functional exercise programs work well as general rehabilitation for those with good body symmetry but not for the 10% of you who fail to see results, time and time again! You may go along to a Pilates class where everyone is given the same functional exercises. You may go to see a personal trainer, and you’d be given functional exercises for weak core muscles or maybe to build overall strength, but again these exercises are generic. These mirror-image-exercises are designed to fit the altered shapes and asymmetry in your body – when the left side does not match the right side and your spinal curves have been altered. Few practitioners and very few trainers and teachers are trained, qualified or even interested in using a structural approach because it takes much longer to train and should involve years of university-level-study in radiology. Carrying excess body weight can also lead to poor posture. Swayback posture develops when the abdominal weight overpowers the weak stabilizing muscles in the lower back. 3 Essential Elements of a Healthy Lifestyle | Posture Doctor A US Study in The American Journal of Medicine found that 62.1% of all US bankruptcies are caused by medical illness. With statistics like that, we can no longer rely on the simplicity of one cause, one effect – in other words, poor physical health (cause) may lead to pain (physical effect), depression (emotional effect), lost productivity (career effect) and eventual bankruptcy (financial effect) for 62.1% of Americans. Well-being In Tom Rath’s rather interesting book, Well-being, he points out that well-being isn’t only about being happy, wealthy or successful. He cautions those of us who are quick to buy programs to get rich or lose weight or sort out our love lives, because when these single-focus programs fail, we may wonder what went wrong. Posture When it comes to correcting our posture, we need a similar approach. If we are to truly apply the best formula for fixing our posture, we need an approach that addresses more than just our physical well-being. Although our physical lifestyles contribute largely to our body postures, it’s essential to address all elements of a healthy lifestyle, if we are to get real results and results that last. Three essential elements of a healthy lifestyle When I work with my posture pupils, I start with a very long eligibility questionnaire designed to identify areas that need more attention, as well as pointing out areas of strengths. I often find certain elements lacking in those pupils not satisfied with their health results to date; those who have been working at fixing their posture for a long time, with little or no results. I like to breakdown these healthy lifestyle elements into three essentials: Physical lifestyle Chemical lifestyle Emotion lifestyle Physical Lifestyle When most people think about their posture they focus on how they sit or stand, what chair or pillow they should buy, or the latest fitness fad to solve all their problems in less than a month. That’s certainly not the very best approach! The second biggest posture mistake, is forgetting your lifestyle! Physical lifestyle is very important, but it’s not everything. The most effective way to address the physical causes of poor posture, is to make daily posture exercise a habit. When you create a healthy posture habit for life, you hard wire it into the brain, until it no longer feels like a chore. You would no more miss your daily Posturecise, than you would miss breakfast, having a shower or brushing your teeth.
